PLEASE READ THE INSTRUCTIONS CAREFULLY:

1. You need to attach a Product Information Sheet to this form, with details on the items being shipped. You can download the Product Information Sheet here. Please read the instructions inside and fill out the details correctly. Incorrect information could lead to shipment abandonement, for which Boxaroo will not be responsible.

2. For security reasons, our warehouse staff will not be able to accept the shipment if this form isn't submitted at least 2 business days before the shipment delivery date.

3. For security reasons, our warehouse staff will not be able to accept the shipment if any duties or taxes are not already paid at the time of delivery.

4. A fee of 199/shipment (199 per shipment) applies for receiving, checking and re-stocking the inventory, and will be applied to your monthly shipping bill.
